Step 1 Read Steps 1 thru 7 and Click Start Web Check-In Button above

Step 2 Completed the online form, making sure the phone number given is the cal back number that contacts you for the next two to three hours.

Step 3 If you DO NOT hear from us within fifteen minutes, please call and report in.

Step 4 A staff member will call within ten minutes to confirm receipt of your check-in. You will receive a second call approximately twenty-five (25) minutes prior to your time to be seen.

Step 5 When you arrive at the clinic, report to the patient service/check-in window. Be sure you bring a photo ID, insurance card and list of your medications.

Step 6 You will be taken to triage and your vital signs and medical history taken.

Step 7 See the medical provider.

Special Note: Patients registering online before or after hours will be contacted and seen during the next normal clinic hours, including Saturday and Sunday. If you do not register after hours and do not here from us the following morning, please contact us to confirm receipt of your registration and approximate call-back time.
